The undersigned hereby (a) declare and confirm that I have been properly informed that the Hotel satisfies the provisions of the law regarding the special healthcare protocols (as provided under the Joint Ministerial Decision 1881/29.5.2020 regarding the special healthcare protocols that will apply to the touristic enterprises in light of the measures against COVID-19) and (b) acknowledge that the Hotel shall not be held liable (for the restoration of damaged or otherwise), as long as the measures provided under said healthcare protocols are complied with. 

Zgadzam się, że w celu ochrony zdrowia publicznego hotel musi na prowadzić aktualny zapis wszystkich gości przebywających w hotelu – imię i nazwisko, narodowość, datę przyjazdu i wyjazdu, dane kontaktowe (telefon, adres e-mail), tak aby komunikacja była możliwa w przypadku koronawirusa w późniejszym terminie. RODO jest przestrzegane przez wszystkich pracowników, a informacje o gościach będą przechowywane w aktach ze względu na ochronę zdrowia publicznego.

I further confirm that I have read and understood and agree to all the above and all the information given and I have personally informed all parties in my booking who agree with the information given and we will all comply with any other related requirements, measures and protocols notified to us by the Hotel. 

Oświadczam i potwierdzam, że zostałem należycie poinformowany, że KOALA HOTEL spełnia przepisy ustawy dotyczące specjalnych protokołów opieki zdrowotnej (zgodnie ze wspólną decyzją ministerialną 1881/29.5.2020 w sprawie specjalnego protokołu opieki zdrowotnej które będą miały zastosowanie do przedsiębiorstw turystycznych w świetle środków przeciwko Covid-19) i uznają, że Hotel nie ponosi odpowiedzialności (za przywrócenie szkód lub w inny sposób), o ile środki przewidziane w tych protokołach opieki zdrowotnej są przestrzegane.

I acknowledge that my personal data and information will need to be used by the Hotel, as prescribed under the abovementioned protocols (and particularly in cases of potential or confirmed infections) and expressly agree thereon. 

Potwierdzam, że ja i współdzielący mojego pokoju nie mają objawów choroby Covid-19, nie jesteśmy w kwarantannie, podejmiemy wszelkie niezbędne środki ostrożności przed epidemią i podróżujemy na własne ryzyko.

I agree that for public health protection, the Hotel must keep an updated record of all Guests staying at the hotel – name, nationality, date of arrival and departure, contact details (telephone, e-mail) so that communication is possible if a coronavirus case is identified at a later time. The GDPR is complied by all staff members and -Guests' information will be kept on file for reasons of public health protection

Ich stimme zu, dass das Hotel aus Gründen des Schutzes der öffentlichen Gesundheit eine aktualisierte Aufzeichnung aller Gäste, die sich im Hotel aufhalten, führen muss - Name, Nationalität, Ankunfts- und Abreisedatum, Kontaktdaten (Telefon, E-Mail), damit eine Kommunikation möglich ist, falls zu einem späteren Zeitpunkt ein Coronavirus-Fall festgestellt wird. Die GDPR wird von allen Mitarbeitern eingehalten, und die Informationen der Gäste werden aus Gründen des Schutzes der öffentlichen Gesundheit in den Akten aufbewahrt.

Συμφωνώ ότι τα προσωπικά μου στοιχεία που αναφέρονται παραπάνω περιλαμβάνονται στο αρχείο προσωπικών δεδομένων που το ξενοδοχείο διατηρεί και επεξεργάζεται σωστά με σκοπό την κατάλληλη εξυπηρέτηση, υποστήριξη και παρακολούθηση της τρέχουσας σχέσης μου με αυτό, αποκλειστικά για αυτόν τον λόγο, διατηρώντας όλα τα δικαιώματα σύμφωνα με τους κανονισμούς της ΕΕ 2016/679.

Sono d'accordo che per la protezione della salute pubblica, l'hotel deve tenere un registro aggiornato di tutti gli ospiti che soggiornano presso l'hotel - nome, nazionalità, data di arrivo e partenza, dettagli di contatto (telefono, e-mail), in modo che la comunicazione sia possibile se un caso di coronavirus è identificato in un momento successivo. Il GDPR è rispettato da tutti i membri dello staff e le informazioni degli ospiti saranno tenute in archivio per ragioni di protezione della salute pubblica.
